Referral process

Referrals for St Leonard’s Hospice in-patient beds must be made by a healthcare professional. This can be a hospital doctor or nurse, or a member of the community team such as a district nurse, GP or Macmillan Nurse.

Referrals should be made using the referral form, available below.

Admissions 
We have increased the number of admissions to our in-patient unit. To make a referral please complete the referral form and email it to SLEHO.spcreferrals@nhs.net.

 

For any Hospice community referrals, including:
  • Hospice@Home
  • Marie Curie (night support)
  • Medical consultation and wider multidisciplinary team discussion
Contact our Single Point of Coordination Service (SPOC) on 01904 777 770. This service is available between 8am and 6:30pm Monday to Friday and 8am to 4pm Saturday and Sunday.
Please can you also ensure any S1 tasks for the Hospice@Home team are sent to SPOC before 4pm each day.
