Referral process

Referrals for all St Leonard’s Hospice services must be made by a healthcare professional. This can be a hospital doctor or nurse, or a member of the community team such as a district nurse, GP or Macmillan Nurse.

All referrals to Hospice services should be made using the referral form available below, however in urgent circumstances please contact our Single Point of Coordination service (SPOC) on 01904 777770
