Make a referral

Referrals for all St Leonard’s Hospice services must be made by a healthcare professional. This can be a hospital doctor or nurse, or a member of the community team such as a district nurse, GP or Macmillan Nurse.

All referrals to Hospice services should be made using the referral form linked below.If you are unsure which service is most appropriate for your patient, please see the eligibility criteria below.

Please note: any referrals made after 4pm will be processed the next day. If you have an urgent referral please contact the team on 01904 777 770
