Referral process

Referrals for all St Leonard’s Hospice services must be made by a healthcare professional. This can be a hospital doctor or nurse, or a member of the community team such as a district nurse, GP or Macmillan Nurse.

All referrals to Hospice services should be made using the referral form available below.

Please note: referrals made after 4pm will be processed the next day. If you have an urgent referral please contact the team on 01904 777 770
