Single Point of Coordination

This telephone service brings together end-of-life care specialists from across the community, rapidly responding to patient and family needs to provide an exceptionally high level of care in the local area.

Services supported by Single Point of Coordination are:

  • York Hospital’s Macmillan Community Palliative Care Clinical Nurse Specialist service
  • St Leonard’s Hospice@Home service
  • St Leonard’s In-Patient Unit
  • St Leonard’s Sunflower Wellbeing Hub
  • St Leonard’s Carer Support service
  • St Leonard’s Bereavement Support service (call our dedicated bereavement phone number 01904 777 760)
  • Marie Curie night support visit referrals and allocations

Our phone lines have a dedicated coordinator responding to calls and referrals between:

  • 8am until 4.30pm from Monday to Friday
  • 8am until 4pm on weekends and bank holidays
  • If you have an urgent query outside of these times the service will be limited
