Completion of the Homeless Palliative Care Project with St Leonard’s Hospice and Changing Lives

The Homeless Palliative Care Service was a partnership project with St Leonard’s Hospice and Changing Lives, which ran from November 2020 – February 2022. The role of Complex Care Coordinator ends with the project.

We are now reviewing the best way forward to enhance our reach to inclusion groups across our catchment area, inclusive of reviewing our approach to delivering service provision.

Please could you communicate this with your team and any service users who may have received support from the Homeless Palliative Care Service.  Service users requiring palliative care support should now be signposted to their GP.
