Actual Salary: £35,745 – £43,044 Pro Rata (Band 6 on the St Leonard’s Hospice Pay Scale)
Hours: 37.5 per week
JOB SUMMARY
This role will lead on optimising patient flow through the inpatient unit, to enable greater responsiveness to our community. You will be the first point of contact for all inpatient referrals and will therefore be required to effectively triage referrals based on clinical complexities and discussions with key system partners.
The role will involve fostering close relationships with key referrers and also promoting the hospice to new referrers so that we can widen our reach to more patients with a palliative care diagnosis.
Supported by the wider multidisciplinary team, the post holder will need to work collaboratively with members across the organisation to optimise our one hospice approach, and fully optimise our internal services to effectively promote onward care in the community.
Accountable to the Head of inpatient services, you will be a credible healthcare professional, who strives for quality and patient centred care in your daily working, ensuring service delivery is evidence based, responsive and aligns to our mission. The role will form part of the inpatient unit leadership team, and thus will be expected to support with positive role modelling, endorsing our values in daily practice and supporting with developing the wider teams confidence in supporting with discharge focus. In addition supporting with line management of identified direct line reports.
In ambition of increasing our reach to see more patients, the post holder will be expected to support the Head of inpatient services with audit cycles, review of patient flow systems to ensure our service offer remains contemporaneous to our community needs, and changes to palliative care response. This will require strong partnership relationship building and a proactive and enquiring nature to seek feedback from referrers in addition to collating the experiences of our patients and their families.
